What Should I Bring to My First Appointment?

Please bring a valid form of identification (driver’s license), insurance card, prescription from your doctor (only if referred), a list of medications you are currently taking, any relevant imaging reports related to your injury or condition, and the full intake paperwork filled out.

What Should I Wear?

Please wear loose, comfortable clothing that you can easily move in and that exposes your problem area.

What Insurances Are Accepted?

AFM Physical Therapy is considered an out-of-network provider with most insurance companies. If you have out-of-network benefits, as a courtesy, we will verify the details of your benefits prior to your first appointment. You will be responsible for the deductible and co-payment/co-insurance when applicable.

AFM Physical Therapy is in-network with Medicare. This means, we are a certified Medicare provider allowing us to see patients who are Medicare beneficiaries.

Can I Still See You If You Don’t Take My Insurance?

Yes! The rates for the initial evaluation and follow-up treatment sessions may vary. AFM Physical Therapy also offers various private payments plans and packages. Please contact us for more information on our fees and payment details.

Do I Need A Prescription To Be Treated?

You do not need a prescription to be treated in the State of New Jersey (unless you are a Medicare beneficiary) . New Jersey is Direct Access state, meaning patients don’t require a doctor’s prescription or referral to receive physical therapy treatment.  

Cancellation Policy

Our number one goal at AFM is to provide the best care for our patients. Failure to keep your scheduled appointments hinders our ability to deliver on this promise. We ask that you show us and other patients trying to get scheduled a 24-hour (or greater) notice for cancellations. Only emergencies or illnesses are excusable. Please call (201-644-8300) or email (info@afmphysicaltherapy.com) with your notification. This will allow us the opportunity to offer that appointment to another patient.

A violation of this policy will result in a $35.00 No-Show/Late Cancel Appointment Fee. The fee cannot be billed to your insurance carrier and will be your direct responsibility. If you arrive late, you will only receive the remaining time in your session.

If you reschedule for another day during the same week, you will not be charged the $35.00 No-Show/Late Cancel Appointment Fee.
